Sesamoiditis: pain under the big-toe joint

Short answer

Sesamoiditis is inflammation of the two small sesamoid bones and their tendons under the big-toe joint. It causes a dull, gradual pain under the ball of the foot on the big-toe side that varies with activity and footwear. ACFAS lists rest, ice, padding, taping and orthotics among the usual treatments. A sesamoid fracture or turf toe can look similar, so persistent pain needs an exam.
